Digital Liability Waiver & Agreements

Release of Liability

Participant and parent/guardian acknowledge and accept the inherent risks associated with martial arts and fitness activities. By enrolling, you release Shogun Tigers Youth Academy & Martial Fitness, its instructors, and staff from liability for injuries sustained during normal participation.

Medical Authorization

In the event of a medical emergency and if a parent or guardian cannot be reached, I authorize the academy staff to seek emergency medical treatment for my child at the nearest appropriate facility. I agree to be responsible for any costs incurred.

Photography Release (Optional)

I grant Shogun Tigers Youth Academy & Martial Fitness permission to photograph and/or video record my child for use in promotional materials, social media, and the academy website. This consent is optional and may be withdrawn in writing at any time.

Cancellation Policy

A 30-day written notice is required to cancel enrollment. Cancellations made without written notice will result in a final month's tuition charge.

Auto-Payment Agreement

Monthly tuition will automatically draft from the payment method used during enrollment. Electronic acceptance of this agreement is required to complete enrollment.
